For the 2019-2020 School Year- Season 4 sports have been cancelled due to COVID-19
Season 4 sports: Girls Volleyball, Golf, Flag Football and Cross Country
For the 2019-2020 School Year, all sports sign ups must be completed at www.familyid.com before the first tryout or practice date. If you have signed up on FamilyID in the past, do not create another FamilyID account. Just renew any information in your existing acccount as necessary and make sure you have checked the sports you are interested in.
Furthermore, a current physical exam must be on file with the Athletic Department. Physicals are valid for 13 months.
Once a student is registered on Familyid.com and has a current physical exam on file with the Athletic Department or Family ID, the student will need a "Golden Ticket" to hand to their coach for the 1st tryout or 1st practice (if the sport does not have tryouts.) You can obtain a Golden Ticket from the Athletic Director.
NOTICE: The requirements for Parent Drivers has changed; Proof of a passed TB test and a fingerprint screening have been added to the Transportation form, auto insurance information, and CDL copy requirements
Dear Athlete/Parent/Guardian,
We are happy that you have expressed a desire to participate in the athletic program at Bancroft Middle School. In order to participate on a team, there are responsibilities and obligations, which you must assume to qualify for and remain a part of the athletic program. Please visit www.familyid.com to register/sign up. Please read the qualifications governing the athletic program before signing the agreements so you are fully aware of your responsibilities. FamilyID will also request specific medical and contact information. Below is a list of what needs to be completed or adhered to.
- Register on Family ID (www.familyid.com), which includes contact and emergency information, medical information including health insurance coverage, citizenship and sportsmanship requirements, obligations, agreements, and more. NOTE: If you have signed up on FamilyID in the past, do not create another FamilyID account. Just renew any information in your existing acccount as necessary and make sure you have checked the sports you are interested in.
- Submit to the Athletic Department or upload on Family ID a copy of a non-expired/current sports physical, from the doctors office, that clears you to fully participate. Physicals expire after 13 months from the exam date.
- Possible adherence to team specific codes of conduct, uniform contracts, practice quidelines, etc.
- A minimum grade point average of 2.0 must be achieved on the quarter report card immediately preceding the beginning of the season. This minimum 2.0 GPA must be maintained for the duration of the sport.
